Massage Addict no longer requires physical or printable health history forms.

If you’ve recently booked your first appointment, please check your email (and spam or junk folders) for two messages:

• Booking confirmation details
• Online intake forms (please complete these before your appointment to ensure a smooth check-in process).

Once submitted, your forms are automatically sent to the clinic and processed in advance of your visit. No need to bring any paperwork with you.

If you don’t see your intake form email or are having trouble completing it, please contact your clinic directly for assistance.
